Appointments:

Routine Appointments $90 per appointment.
*****There is a maximum appointment time limit of 25 minutes.

Unlimited Plan $1,100 per every 365 days.

*****The expiration date for this plan is calculated as 11:59 P.M. on date of payment + 1 year + 1 day.  For example, if a payment is made at 10:00 A.M. on June 11, 2018, then the expiration date for that patient's Unlimited Plan would be 11:59 P.M. on June 12, 2019.

Financial


Complete payment is due before the appointment unless otherwise specified by Men's Health Care. Men's Health Care will not provide medical care to patients who do not have an up-to-date (zero) balance. Failure to maintain an up-to-date (zero) balance will result in rescheduling of a patient's appointment until a time of their choosing when they are able to maintain an up-to-date (zero) balance.

Patients are permitted to use any combination of payment with cash, credit card and/or debit card to tender their payments. For example, a patient may pay his fee for his appointment using cash to pay half of his fee and credit card to pay the other half of his fee (or any combination thereof). 

Men's Health Care, generally, will not charge patients for follow-up appointments for certain conditions (i.e. unresolving colds, etc.) or for review of abnormal test results. Please note that for follow-up appointments for certain conditions (as specified above) that only the condition for which follow up is required will be addressed. Please contact Men's Health Care for further details.

Men's Health Care accepts cash, credit cards and debit cards. Credit cards accepted include American Express, Discover, JCB, MasterCard, UnionPay and Visa. With the exception of cash payment for medical records, we do not accept cash in the form of change (coins). We only accept bank notes (paper money). We do not accept checks, money orders, traveler's checks or any other forms of payment. Men's Health Care is able to offer affordable prices by avoiding unnecessary fees which would otherwise be passed onto its patients.

Men's Health Care may be willing to accept barters in the case that one does not have the financial resources to pay for medical care. Examples of barters include goods, services, time and other exchanges of non-monetary commodities.

Men's Health Care provides corporate discounts as well as group discounts. Please contact Men's Health Care for further details.

A patient's fees paid to Men's Health Care does not provide entitlement to one for prescriptions, diagnoses or anything besides time (as clinically indicated and as permitted by office scheduling) with A. A. Emami, D.O., M.D., F.H.U., F.A.C.O.I. This is a full service medical practice that attempts to address all of the medical needs of its patients as dictated by A. A. Emami, D.O., M.D., F.H.U., F.A.C.O.I.'s scope of practice. There is a maximum appointment time limit of 40 minutes for annual comprehensive physical examinations (physicals) and 25 minutes for appointments made for any other reason.


Prescriptions


Men's Health Care offers Quick Rx:

For prescriptions refills that may not require an appointment, this service saves time and money. There is never a charge for prescriptions which are provided in association with an appointment.

This service is available for a fee of $40 per instance and a $2 postage/shipping fee (if a request is made for the prescription(s) to be mailed). There is no charge for this service for patients enrolled in the Unlimited Plan. Payment may be made in person if a patient chooses to pick up the prescription in-person or over the phone if a patient chooses to have the prescription called, faxed or mailed to a pharmacy of their choosing. Please note, certain laws may prohibit Men's Health Care from calling pharmacies with a prescription because certain prescriptions can only be written (not called or faxed).

Please allow up to 24 hours for processing of Quick Rx requests.

This service is generally available to all established patients who are current patients as witnessed by their having had an appointment within the last 365 days. A $40 fee and a $2 postage/shipping fee (if a request is made for the prescription(s) to be mailed) will be assessed for each instance Quick Rx is used. There is no charge for this service for patients enrolled in the Unlimited Plan. The fee is for prescriptions for six-month supplies (one month's worth of medication and five refills). Please note, the fee being paid is for the written prescription and does not cover the cost of medications once the prescription is filled at a pharmacy. Only approved medications are eligible for the Quick Rx service. Generally, a patient may receive a 6-month supply for all of their approved (non-controlled) medications. The $40 fee is charged per instance (not per medication) so that a patient may have all of their approved medications filled at one time (see example A and B below). Please note, although a patient may have a prescription for all of their medications to be filled, it does not mean that they all need to be filled at one time. Please also note, prescriptions will only be issued in an on-cycle manner (see example C below). A request can be made to the pharmacy/pharmacist to refill certain medications at a later date. Please contact Men's Health Care for further details and/or details about specific medications.

Example A:

If Mr. Ken makes a Quick Rx request to receive a prescription for his blood-pressure medication, his cholesterol medication, his thyroid medication and his diabetes medication on 06/01/2016, he will be charged only $40 on 06/01/2016. Please note, if Mr. Ken takes his prescription to the pharmacy and he requests that he only has his prescriptions for his blood-pressure medication and his cholesterol medication to be filled at that time, he can make a request to the pharmacy/pharmacist to keep his prescriptions for his thyroid medication and his diabetes medication to be kept on file so that they can be filled at a later date.

Example B:

If Mr. Ken makes a Quick Rx request to receive a prescription for his blood-pressure medication and his cholesterol medication on 06/01/2016, he will be charged $40 on 06/01/2016. If Mr. Ken then makes another Quick Rx request to receive a prescription for his thyroid medication and his diabetes medication on 07/01/2016, he will be charged another $40 on 07/01/2016.

Example C:

If Mr. Plainview makes a Quick Rx request to receive a prescription for his blood-pressure medication on 06/01/2016, he will be charged $40 on 06/01/2016. If Mr. Plainview makes another Quick Rx request on 07/01/2016 for the same blood-pressure medication, no prescription will be provided and no fee will be collected. On 07/01/2016 Mr. Plainview made a Quick Rx request for a six-month supply of his blood-pressure medication after having received a prescription for a six-month supply for his blood-pressure medication on 06/01/2016. Thus, Mr. Plainview is off-cycle. Mr. Plainview will be eligible to make another Quick Rx request to receive a prescription for his blood-pressure medication on or after 11/01/2015.

Generally, for most (non-controlled) medications, A. A. Emami, D.O., M.D., F.H.U., F.A.C.O.I. will provide prescriptions for a 6-month supply (one month's worth of medication and five refills) of medications. Please contact Men's Health Care for further details and/or details about specific medications.

Generally, for most medications, A. A. Emami, D.O., M.D., F.H.U., F.A.C.O.I. recommends generic medications (when a generic equivalent is available) since they are usually as clinically effective as brand-name medications but at a fraction of the cost. Patients generally have the option (at the pharmacy) to have their prescriptions filled as the brand-name medication or as its generic equivalent (when a generic equivalent is available). The pharmacy will be able to provide you more information (such as pricing) before you decide on proceeding to have a prescription filled as the brand name or as its generic equivalent (when a generic equivalent is available). Please note, for newer medications (usually due to patent laws) there may not be a generic equivalent available.

Men's Health Care complies with all local, state, and federal laws. Prescriptions will be called to pharmacies upon the patient's request. Written prescriptions will be issued only once. It is the patient's responsibility to ensure safekeeping of the prescription from any form of loss. In the case that a prescription is lost, another will not be issued for a length of time at the discretion of Men's Health Care.

Men's Health care does not prescribe medicinal marijuana.

Men's Health Care will provide prescriptions for bona-fide medical issues. Thus Men's Health care is not a steroid clinic otherwise known as an "anti-aging" clinic.

Men's Health care does not provide "anticipatory" prescriptions for acute issues that have not occurred. One example of an "anticipatory" prescription is a prescription for antibiotics to have on hand in case the patient should become ill during a special event (i.e. a vacation, a wedding, etc.).

Men's Health Care makes every effort to provide its patients with free medication samples whenever they are available. Please note that no pharmaceutical company and/or the samples they may dispense influence the clinical decisions made at Men's Health Care. Furthermore, Men's Health Care and its members have no direct or indirect financial interests in pharmaceutical companies.

Men's Health Care realizes that there is a national epidemic of prescription drug abuse and illegal diversion. Men's Health Care rarely (and on a case-by-case basis) issues prescriptions for certain controlled substances. For patients who suffer from chronic pain, we recommended establishing care with a Doctor who specializes in Pain Management or Pain Medicine for this particular medical issue. For individuals looking to profit from (illegal) diversion of controlled substances or the attempted (illegal) diversion of controlled substances, Men's Health Care works in lockstep with the United States Food and Drug Administration (F.D.A.) the United States Drug Enforcement Administration (D.E.A.) state drug-enforcement agencies, as well as local drug-enforcement agencies. Men's Health Care maintains a low threshold in reporting individuals seeking to profit from the (illegal) diversion of controlled substances. Men's Health Care is not a "pill mill."

Men's Health Care regularly monitors The Arizona Board of Pharmacy's Controlled Substances Prescription Monitoring Program (C.S.P.M.P.) database.

All prescriptions will be provided at the discretion of A. A. Emami, D.O., M.D., F.H.U., F.A.C.O.I. according to clinical appropriateness and according to the patient's medical needs.


Test Results


For normal test results or for test results with minor abnormalities, A. A. Emami, D.O., M.D., F.H.U., F.A.C.O.I. will call the patient with the results. If the patient gives permission for Men's Health Care to leave voice messages, one will be left indicating what tests are normal or have minor abnormalities.

For test results with major abnormalities, A. A. Emami, D.O., M.D., F.H.U., F.A.C.O.I. will call the patient and advise him to schedule an appointment to review the test results and to provide medical counseling. There will be no charge for this appointment.


Medical Records


Requests for medical records will be transmitted upon signature of The Men's Health Care Release of Medical Information and Medical Records Request document by the patient or the patient's authorized representative. Please allow up to one week for processing and transmission of medical records. A processing fee of $15, a copying fee of $0.50 per page, and a $2 postage/shipping fee (if a request is made for medical records to be mailed) will be assessed. Medical records will not be transmitted by Men's Health Care until payment is tendered for the medical records.

Men's Health Care does not charge a fee for medical reports to be transmitted to another Doctor for whom the patient has been referred by Men's Health Care. The medical report will be transmitted upon signature of a release of records document by the patient.

Men's Health Care does not charge a fee for requests for test results made within 1 week of A. A. Emami, D.O., M.D., F.H.U., F.A.C.O.I.'s receipt of the test result.

Men's Health Care does not accept health/medical insurance. Men's Health Care is able to offer affordable prices to its patients by avoiding unnecessary fees and unnecessary staffing due to health/medical insurance requirements. By eliminating this overhead, Men's Health Care is not required to pass this cost that would otherwise be passed onto its patients. If a patient of Men's Health Care does have health/medical insurance and if he requests a medical documentation report for his appointments to submit to his health/medical insurance company for (tentative) reimbursement, a medical documentation report will be provided at a cost of $55 per report per appointment. Patients are advised to notify Men's Health Care prior to the start of their appointment if they will be making a request for a medical documentation report. A medical documentation report will be transmitted upon signature of The Men's Health Care Release of Medical Information and Medical Records Request document. Please note, no revisions will be made to this document if it does not meet documentation and/or billing benchmarks as set forth by the health/medical insurance company. Men's Health Care makes no guarantee that any of its patients will be reimbursed by their health/medical insurance company.

Men's Health Care does not participate with insurance companies regarding pre-authorization forms or any other forms they may request to have completed.


General


Patients who are dismissed from Men's Health Care will not be issued a refund. Men's Health Care reserves the right to dismiss any patient, at any time, for any reason. In case a patient is dismissed, he will have 30 days to obtain care/seek care from a new Doctor. The 30 days will begin on the day that a certified letter is sent to the patient via postal mail.

Men's Health care is not affiliated with (and does not have any financial relationships) any other entity. Other entities include (but are not limited to) health/medical insurance companies, home health/medical suppliers, hospitals, laboratories, other Doctors, pharmacies and radiology/imaging centers.

Men's Health Care does not participate in any disability program including workers' compensation.

Recording of any kind (audio, photo, video and/or by any other means) is strictly prohibited. Discovery of such an act will result in immediate dismissal as a patient from Men's Health Care. Any violation of patient privacy will be pursued in a court of law.

Men's Health Care does not schedule appointments for patients with other Doctors and/or for tests. It is the patient's responsibility to schedule appointments with other Doctors and/or for tests.

Men's Health Care does not solicit test results/reports from other Doctors. It is the patient's responsibility to ensure that test results/reports from other Doctors are forwarded to Men's Health Care.

Theft of any equipment/object owned by Men's Health Care will result in immediate dismissal from Men's Health Care. Any theft will be reported to local law authorities and will be prosecuted to the fullest extent of the law.

In the case Men's Health Care determines that a patient requires emergency treatment, a higher level of care or hospitalization, the patient will be notified that an ambulance (emergency-medical transportation) will need to be called (if Men's Health Care deems a patient to be unstable for transport to the hospital by other means). If the patient agrees to stay at Men's Health Care until an ambulance (emergency-medical transportation) arrives, an ambulance (emergency-medical transportation) will be called and he will be assessed by emergency-medical responders and (likely) transported to the hospital's emergency department for further assessment. If the patient does not agree to stay at Men's Health Care until an ambulance (emergency-medical transportation) arrives, an ambulance (emergency-medical transportation) will not be called and he will be fully informed of the risks of not being assessed by emergency medical responders. Men's Health Care respects patient autonomy and is not of the practice of holding patients against their will as this is medically unethical unless he presents a threat to himself or others. In case a patient leaves Men's Health Care without agreeing to being assessed by emergency-medical responders, he will be asked to sign the Men's Health Care Refusal to Permit Further Treatment and Release from Responsibility for Discharge document (prior to leaving) absolving Men's Health Care of all responsibility as the patient will have been informed of all of the risks of his decision. In case a patient refuses to sign the Men's Health Care Refusal to Permit Further Treatment and Release from Responsibility for Discharge document, one will be signed by A. A. Emami, D.O., M.D., F.H.U., F.A.C.O.I. in his stead.


Scheduling/Missed Appointments


Patients who are more than 15 minutes late for their appointment may have their appointment canceled or rescheduled by Men's Health Care.

Men's Health Care will make every attempt to address all of its patients' medical concerns, issues and needs. When a patient schedules an appointment, he will be asked the precise reason(s) for scheduling the appointment. In an attempt to provide prompt and timely medical care to the patients of Men's Health Care, A. A. Emami, D.O., M.D., F.H.U., F.A.C.O.I. will only address the precise reason(s) stated by the patient for scheduling the appointment and any associated or incidental findings during the appointment. If the patient mentions additional medical concerns, issues and needs that were not mentioned as part of the reason for scheduling the appointment, he will be advised to schedule another appointment. This is done as a courtesy to other patients, in order to provide prompt and timely medical care for their appointments as well. Patients are advised the receptionist who schedules appointments is a trained professional who does not belittle, judge or shame patients while maintaining strict patient confidentiality. Patients are encouraged to fully disclose the reason(s) for scheduling their appointment and be reassured that Men's Health Care will dedicate all of its efforts towards seeking a cure, a diagnosis, a resolution and/or a treatment.

Men's Health Care will assess a fee of $55 for each appointment that is missed (no-show) or for each appointment that is rescheduled within 24 hours of the patient's scheduled appointment. If a patient has a Monday appointment but cancels his appointment on Friday after the time (hour) of his appointment, he will be assessed a fee of $55. Exemptions to the fee will be made on a case-by-case basis only for bona fide and/or objective reasons. Patients who habitually miss appointments or who habitually reschedule appointments within 24 hours of their appointment may be subject to dismissal from Men's Health Care.


Legal


Men's Health Care will report all instances of forgery and/or misrepresentation to the United States Food and Drug Administration (F.D.A.) the United States Drug Enforcement Administration (D.E.A.) state drug-enforcement agencies, as well as local drug-enforcement agencies. We will also seek the maximum monetary amounts allowed under statute for any and all instances of forgery and/or misrepresentation.

The undersigned understands that Men’s Health Care will consider any negative online reviews as a breach of contract which will initiate dismissal of a patient from Men’s Health Care upon discovery of the online review(s).  The patient will have 30 days to obtain care/seek care from a new Doctor.  The 30 days will begin on the day that a certified letter is sent to the patient via postal mail.  Men’s Health Care will seek maximum damages for all cases of defamation whether it is conveyed via libel (published), slander (spoken) or by any other means.
